Social Determinants of Health Program

Our Social Determinants of Health Program helps connect immigrants and newcomers with resources and social services to meet their needs.

Our CCHBs and family physicians can help identify social determinants of health needs in our patients. 

Our SDH Coordinator can help make referrals to organizations and community resources specific to the patient’s needs.
